CO₂ Laser After 40: What It Can Actually Do for Ageing Skin

What actually happens to our skin after 40?

From our 40s onwards, changes in collagen, elastin, cell turnover and hydration can become much more noticeable. Hormonal changes during perimenopause and menopause can also have a significant effect on the skin.

You may start noticing:

  • Fine lines becoming more established
  • Crepey skin, particularly around the eyes
  • Loss of firmness
  • Uneven skin texture
  • Enlarged or more visible pores
  • Pigmentation and sun damage becoming more apparent
  • Skin looking duller and less reflective
  • Old acne scarring becoming more noticeable

This is one of the reasons I believe treating the quality of the skin itself becomes so important as we age.

So what does fractional CO₂ laser actually do?

CO₂ laser is a resurfacing treatment. Fractional technology delivers tiny columns of laser energy into the skin while leaving surrounding areas untreated, allowing the skin to repair itself.

This controlled process stimulates a wound-healing response and encourages collagen remodelling.

Over the weeks and months following treatment, this can lead to improvements in concerns such as fine lines, wrinkles, skin texture, sun damage, pigmentation, enlarged pores and certain types of acne scarring.

It can also be particularly useful for areas where the skin has become crepey, such as around the eyes.

Is CO₂ laser a replacement for filler?

Not necessarily — they do completely different jobs.

Dermal filler is primarily used to restore or create volume and alter facial contours. CO₂ laser is focused on skin quality and resurfacing.

For many of the women I see in their 40s, 50s and beyond, the conversation has shifted away from simply asking, “What can I inject?” towards asking, “How can I improve the actual quality of my skin?”

Sometimes the right answer may involve a combination of treatments. Sometimes it may not involve filler at all.

A good consultation should determine what is actually causing the concern before deciding on a treatment.

What results should you expect?

CO₂ laser isn’t an overnight treatment.

There is an initial healing period, and the skin can look red, swollen and feel rough or dry as it recovers. Peeling and flaking can occur, and the exact downtime varies depending on the intensity of treatment and the individual.

The longer-term changes happen gradually as collagen remodelling takes place.

The goal isn’t to make you look like a different person. It’s to improve the texture, tone and overall quality of your own skin.

And importantly, no responsible practitioner should promise perfect skin or guarantee a particular result.

Is everyone suitable for CO₂ laser?

No — and this is really important.

CO₂ laser is a powerful treatment and suitability needs to be assessed properly. Your skin type, pigmentation risk, medical history, medications, previous treatments, lifestyle and expectations all need to be considered.

Some people may be better suited to a different treatment altogether.

This is why I don’t believe CO₂ laser should simply be booked because you’ve seen an impressive before-and-after photograph online. Consultation and correct patient selection matter just as much as the technology itself.
